Preparing for Your Visit
We want your experience with us to be a positive one, both clinically and administratively. We can help you prepare for your visit and we can help you navigate your insurance and payment options.
Follow these easy steps in preparing for your visit.
Step 1 – Verify insurance coverage. Contact your insurance plan or review your plan documentation to confirm the benefits offered by your health plan, including co-payment amounts.
Step 2 - Determine if a referral is necessary. When reviewing your insurance information, determine if a referral authorization from your primary care provider is necessary.
Step 3 - Register with us online.
Step 4 – Schedule an appointment with us online, or do it over the phone at 952-832-8110. Have your insurance information ready.
Step 5 - Read your privacy rights and let us know if you have questions about these rights.
Step 6 – Be prepared to spend 30 to 90 minutes with us, especially if this is your first visit. We want to spend the time necessary learning about you and your vision needs. Bring a list of current medications and your glasses or recent glasses prescription.
Step 7 – Be prepared to pay your co-payment at your appointment. We accept Visa, Mastercard, American Express, and Discover.
Step 8 - Expect a call from us to confirm your appointment. Contact us if you need to reschedule.
